CHU Forms and Permission Slips
Each student attending CHU needs to complete the three forms below to register for 2009-2010 activities and events .
1. "REQUIRED" Medical Consent/Permission Slip: permission slip for general in town events for 2009-2010 school year and consent for medical treatment in an emergency. This DOES NOT need to be notorized!

2. "REQUIRED" Family and Student Information: so you will receive mailings, email, and be in the phone directory and database
